You can access the portal by going to OneChartPatient.com or by downloading the Nebraska Medicine app.

Medical records requests
Patient records will only be provided through one of the following options:
- The One Chart | Patient portal
- Mailed to your home
- Emailed (The email will be encrypted. You will receive a password in a separate email message)
If you would prefer to manually request your medical records, or are requesting records for someone else, please follow our request records by mail process.
Right to Request Amendment
You may request your health information be amended if you feel the information is not correct. Your request must be in writing and provide rationale for the amendment.
