Providing comprehensive services for the elderly
Nebraska Medicine provides comprehensive care for older adults that focuses on function and independence. We provide a full continuum of care from outpatient care to rehabilitation, nursing home and home care.
Our Geriatric Medicine clinic follows the Patient Centered Medical Home (PCMH) team approach to primary care. We partner with patients, families and the community to support and promote health and well-being. As a patient, you will regularly be in touch with a nurse care coordinator to stay informed on health and wellness screenings and details regarding personal health conditions.
We also have psychiatrists whom specialize in geriatric mental health to evaluate and treat cognitive disorders as well as specialists who focus on the treatment of diseases like dementia, Alzheimer's and Parkinson's.

We are located within the Home Instead Center for Successful Aging, which is focused on helping senior citizens live better and healthier lives. This free standing center offers care across the continuum for mind, body and spirit. It is the only program tied to an academic medical center in the region.

The Geriatric Medicine clinic is located on the north side of the Home Instead Center at 38th Avenue and Leavenworth Street. Providers focus on the whole person with geriatric psychiatry, nursing, pharmacy, physical therapy and exercise science all in one place.
The center also offers a state-of-the-art gym, EngAge Wellness, that is equipped with ergonomic fitness equipment that caters to the unique needs of older people. For a tour, please call 402.552.7210.
What to expect
We provide three types of patient care within the Geriatrics Medicine clinic including primary care, medical consultations or second opinions, and geriatric assessments. The minimum age to schedule a primary care or consultation appointment in our clinic is 65.
Primary Care
Primary medical care can be provided for persons aged 65 and older, who reside in the Omaha area. Your health care will be managed by a care team, consisting of a physician and a nurse practitioner. These highly- skilled, geriatrics professionals work closely together to insure that you receive the best quality of care possible. After your initial appointment with the physician, you will alternate seeing the physician and the nurse practitioner on all future visits.
Medical consultation or second opinion
Medical consultation is a service that provides a second opinion and treatment for a medical condition for persons aged 65 and older, who do not receive their primary medical care in our clinic.
Geriatric Assessment Program
The Geriatric Assessment Program provides a comprehensive evaluation of older persons who are experiencing memory loss and/or a complex of health problems that rob them of their ability to perform essential day-to-day activities. The assessment includes physical exam, a review of medications; cognitive, functional and mood evaluations, and collaboration with the patient's family or significant support persons. A detailed treatment plan is developed and implemented with the patient, the family caregivers and the person's primary care physician.
