I believe I have some pelvic floor weakening and possible prolapse, rectocele/cystocele. Would it be safe to get pregnant again, or should I first be seen for an evaluation?

It would depend on the degree of weakening you think you are experiencing and how long it has been since you delivered. The pelvic floor takes time to re-strengthen after delivery. Physical therapy is often the best option for strengthening between pregnancies for women with a degree of prolapse who desire additional children. Very few providers recommend surgical intervention until childbearing is completed. A urogynecology evaluation would be a great option to assess your current pelvic floor status.
