Official Rules
1. NO PURCHASE NECESSARY TO ENTER OR WIN.
2. TIMING. The Employee Social Sharing Drawing (the “Drawing”) begins on Sept. 25, 2020, at 10:00 a.m., Central Standard Time, and ends on Oct. 2, 2020, at 10 a.m. Central Time (the “Entry Period”).
3. ELIGIBILITY. The Drawing is open to all recipients of the nursing virtual open house email who register in advance of the event (on or before Oct. 2, 2020 at 10 a.m.) or an alternate means of entry, as described below, to Nebraska Medicine (“Sponsor”), and are at all times during the Entry Period and on the applicable Drawing Date (as defined below): an email recipient (a “Participant”). This Drawing is subject to all federal and state laws, rules, and regulations, and is void where prohibited.
4. HOW TO ENTER. During the Entry Period, there are two (2) ways to enter the Drawing, as outlined below.
(a) Zoom Form Entry: A Participant may electronically submit a completed registration form to Sponsor by following all directions on this link: https://unmc.zoom.us/meeting/register/tJEpceugqj0oGdwBy-Jq9zB7ZOpzOocWcMBe
(b) Alternative Means of Entry. A Participant may enter the Drawing via U.S. Mail without completing and electronically submitting the above-referenced entry form by hand printing his or her name, address, city, state, ZIP code, and daytime and evening telephone numbers (including area code) on a 3" x 5" card, and mailing the card to Sponsor at the following address: Nebraska Medicine, Attention: The Nursing Virtual Open House Drawing, Lindsay Nolan, 111 N. 175th St., Suite 20201, Omaha, NE 68118.
(c) Only one Drawing entry per Participant is allowed, whether that entry is submitted electronically or by U.S. Mail.
5. DRAWING. Sponsor will conduct one random drawing on or about Oct. 3, 2020 (for completed registrations submitted between Sept. 25, 2020 and Oct. 2, 2020). On Drawing Date, Sponsor will select two of the Participants entered in the Drawing to win a $50 Amazon gift card. The prize winner will be notified during the live event or by overnight or U.S. Mail and/or telephone or email on or about Oct. 3, 2020. The prize winner may be required to sign an affidavit of eligibility and release, a publicity release where legal, and a federal and state tax release, and return these documents, properly executed, via fax, within 48 hours of notification, and the original documents within 7 days of date of notification via regular mail. The occurrence of any of the following events will result in a Grand Prize winner’s disqualification, in which case the disqualified winner’s Grand Prize may be awarded to a potential alternate winner at Sponsor's discretion: non-compliance with the aforementioned time frames and/or with any of these Official Rules; Sponsor's inability to contact a Grand Prize winner within a reasonable period of time following the applicable Drawing Date; or, a Grand Prize's or Grand Prize notification's return as undeliverable. Allow 6-8 weeks from the date declared a Grand Prize winner for delivery of the Grand Prize via U.S. Mail. Limit one (1) Grand Prize per Grand Prize winner.
6. ODDS. Odds of winning a the Grand Prize are based on the total number of eligible entries received during the Entry Period.
7. LIMITATION OF LIABILITY. By entering this sweepstakes, Participants waive all right to and hold Sponsor harmless from, any claim, liability, loss, damage (including punitive, incidental, and consequential damages), or expense (including attorneys' fees) arising out of or in connection with participation in this sweepstakes or the acceptance, use, or misuse of a Grand Prize. SOME JURISDICTIONS DO NOT ALLOW THE LIMITATIONS OR EXCLUSION OF LIABILITY FOR INCIDENTAL OR CONSEQUENTIAL DAMAGES, SO THE ABOVE MAY NOT APPLY TO YOU. Sponsor will not be responsible for: late, incomplete, or incorrect entries; a Participant's failure to receive a prize notice due to a Participant's spam, junk e-mail, or other security settings or a Participant’s provision of incorrect or otherwise non-functioning contact information; technical, hardware, or software malfunctions, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, garbled, or delayed electronic communications whether caused by the sender or by any of the equipment or programming associated with or used in this sweepstakes; by any human error which may occur in the processing of the entries in this sweepstakes; or any typographical, technological, or other error in the publishing of the offer, administration of the sweepstakes, or announcement of a prize. If, in the Sponsor's sole opinion, there is such an error, or there is any suspected evidence of tampering with any portion of the sweepstakes, or if technical difficulties (including viruses and bugs) compromise the integrity of the sweepstakes, Sponsor reserves the right, in its sole discretion, to cancel or modify this Drawing in a manner deemed appropriate by the Sponsor. In the event of termination, the prize winner will be selected from among all eligible entries received as of the date of termination. In the event a dispute arises as to the identity of a potentially winning entrant, entries will be declared made by the name on the entry form.
8. GRAND PRIZE. The prize consists of two (2) Amazon gift cards. Total value of prize: 50 USD each. Each prize consists of only those items specifically listed as part of the prize. No substitution or transfer of a prize is permitted by a prize winner. Sponsor reserves the right to substitute a prize of equal or greater value. A claimed prize will be awarded. If a prize winner is disqualified for any reason, Sponsor will award the disqualified winner's prize to an alternate winner by random drawing from among all remaining eligible entries. Only three (3) alternate drawings will be held, after which such prize will remain un-awarded.
9. CHOICE OF LAW AND FORUM. Participants agree that all matters arising out of or relating to this sweepstake and these Official Rules are governed by, and construed in accordance with, the laws of Nebraska, without giving effect to any of its conflict of laws provisions thereof. Participants further agree that any legal suit, action, or proceeding arising out of or relating to this sweepstake and these Official Rules shall be brought exclusively in the applicable federal or state courts located in Douglas County, Nebraska.
10. PRIZE WINNER. For the name of the prize winner, available after Oct. 5, 2020, as applicable, send a self-addressed, stamped #10 envelope to: Nebraska Medicine, Attention: The Nursing Virtual Open House Drawing, Lindsay Nolan, 111 N. 175th St., Suite 20201, Omaha, NE 68118.
11. PUBLICITY. By entering the Drawing, the prize winner hereby authorizes Sponsor to announce his or her full name, photo, and likeness and/or statements made by the prize winner on NebraskaMed.com, as well as to publish a post to all social media accounts including without limitation Facebook, Instagram, Twitter, Snapchat, any other social media platforms, and any other forms of media.
12. FACEBOOK DISCLAIMER. In no way is this Drawing sponsored, endorsed, administered by, or associated with Facebook, and each Participant completely releases Facebook from any and all claims related to or arising from this Drawing.
